Who we're looking for
We are looking for 6 part-time Care Coordinators, with a minimum Diploma in Mental Health qualication, Graduates welcome to apply! If you enjoy variety in your day, and working closely with other service delivery professionals, you might be exactly the person we're looking for!

As part of our Family Engagement Team in Toowoomba and Ipswich Qld, EACH is looking for a Care Coordinator to provide care coordination to children and youth, and their carers and families within the Darling Downs and West Moreton catchments, making a huge difference to the lives of those navigating the mental health system.

What you'll be doing
The role of the Care Coordinator will be to act as the primary contact for children, young people and their families who are, or are at risk of, experiencing a severe and or complex mental health condition. You will work within the step up step down approach to ensure children and young people and their families are aware of and are supported in their journey if they are experiencing severe mental health concerns and ensure the family have access to the support they need.

Who you'll be working with
You will work under the direction of the Clinical Lead and alongside other health professionals to provide youth friendly mental health services within the area
